Welcome to Elite Open School!
The following are the documents we need to process your enrolment in our school. Enrolment processing fee needs to be paid, along with the documents, to commence the enrolment.

Please send a copy of these documents with this registration form.
- High school transcripts* (中学校、高校の成績証明書)
- Proof of residency (ie. Gas or electric bill)– 住民票
- Elite’s authorization form(FERPA)**
- Immunisation card or waiver**
- Birth certificate (住民票)
- Guardian’s ID (住民票)
* For international students, high school transcripts need to be translated (and notarized). They will also need to provide more information regarding their schools and courses.
** The form should be provided by EOS for student/guardian to sign.
Submit enrollment documents through one of the following:

- Or hand deliver them as you come for the initial interview with the school official(s).

*留学生は、高校の成績証明書の翻訳（公証）が必要です。また、学校やコースに関する更に詳細な情報提供を必要とされる場合もあります。
**EOSから提供されるフォームに、学生/保護者の署名が必要です。
